Do I need a prescription from my doctor?
A doctor’s referral or prescription is typically not required which allows you to access our services without having to see your physician first. Contact our office if you have any questions about your insurance plan’s requirements.
Are you in network with my insurance?
FreeForm is only in-network with Blue Cross Blue Shield PPO plans and participates with Medicare. We also work with Worker’s Compensation if you have a claim number. If you have an Auto Insurance claim, we charge the self-pay rate to you at time of service and provide documentation for you to address reimbursement with the auto insurance company.
What if FreeForm isn’t in-network with my insurance or I don’t have any insurance coverage?
We offer flat-rate pricing for Physical Therapy sessions. $200 for initial consultations and $175 for follow up visits. We can provide documentation (typically, a superbill) to submit to your insurance towards your out-of-network benefits. If you have an Auto Insurance claim, we charge the self-pay rate to you at time of service and provide documentation for you to address reimbursement with the auto insurance company.
I have insurance but I have no idea what all of these terms mean. Can you explain it to me?
Of course! Understanding the difference between deductibles, co-insurance, and out of pocket maximums can be very confusing. Our team can get a quote on your plan, explain each term and what that means to you financially before you commit to service.
How many visits will I need?
Each patient’s condition is different. Together with your physical therapist and referring physician, we will develop a plan of care that is right for you. A plan which includes the number of visits will be determined during your first visit and explained to you by your physical therapist.
How long do sessions usually take?
On average, 50-60 minutes per session
What should I wear?
We recommend that you bring comfortable, loose fitting clothing with a pair of athletic or comfortable shoes.
Your website talks about “boutique-style” therapy. What do you mean by that?
FreeForm models its practice on providing attentive, personalized treatment. Expect a plan that consists of a personalized session with a licensed Physical Therapist for the duration of your appointment.
Do you have parking?
For our Logan Square location, metered street parking is plentiful in the area. However, we do have dedicated parking spaces located in the lot on Fullerton Avenue just east of Francisco Street (across the street from the Sherwin Williams store). These are available on a first come, first served basis. Please only use spaces designated for FreeForm.
Lincoln Park - there is free, off-street parking available at this location.
